  hey man, great job but there are a few points to consider : 1- the design is not identical the outline of the form is larger the button color is different . 2- the form has no validation at all when I write any thing (words ,minus numbers,) it just give me today's date. 3- even if I calculate my age the days are not accurate.

  it i good but in small screen (mobile) the desing is not good and you can add animation when you move to next step.
