Your solution looks great!
I have a suggestion:
- Try using some validations for the email and phone number fields. Currently, the user can type literally anything, like a number on the name/email fields or letters on the phone field. As long as they are not empty, it's possible to go to the next step.
Each field should have a different validation, because it's a different type of information.
