1-the slider background image has not the same height of the form you can use this code the fix it : background-size: cover; 2-the yearly prices in plan selection are not included . 3-their is other issue is that you don't provied any js validation at the inputs filed and in the selection of plan the user can skip without select a plan

Thank you for your feedback. I’ve already fixed your first and second comments but I'm still having trouble correcting the third.
I will open an issue on github for this bug and try to fix it in the next coming days.
