Multi stage form

I'm calling time on this one - the final modal design doesn't match the brief, but I really wanted to focus on React and CSS with this one (I wanted to do it as a modal rather than part of the form).

I'd really welcome any thoughts on:

• How to improve my code structure
• Any basic errors or React newbie mistakes
• Anything else you can see!
