I have created this using sass/scss to make it responsive for multi screen sizes. I am new to sass/scss, please feel free to comment if you find anything can be written in better way for better designs and make the code reusable.

Also I didn't manage to display invalid message, and at the moment if I click button the confirm button, the form section changes to 'completed' display without validation.. Any help will be appreciated to solve this!! Thank you:)
