Your solution differs slightly from the project requirements. Firstly, the style is a bit different from the design. Secondly, the solution doesn't display a toast notification after successful form submission, and form validation messages are missing. Lastly, the form cannot be completed using only the keyboard adding a tabindex to each input element will help with that.
